Month 4 of 12

4 Months Old

Month four often brings the first real laugh and a big jump in hand-eye coordination — reaching and grabbing start to look intentional.

Last reviewed: August 8, 2026

Four months is a coordination milestone month: reaching, grabbing, and bringing objects to the mouth all start clicking into place close together, and many parents describe this as the point where play starts feeling interactive rather than one-directional. It's also a commonly discussed age for early sleep regressions, driven by genuine developmental changes in sleep cycle structure rather than anything going wrong.

Motor & Physical

  • Pushes up to elbows or straight arms in tummy time
  • May roll from tummy to back
  • Brings hands together and to their mouth
  • Holds and shakes a toy placed in their hand

Cognitive

  • Reaches for and grabs toys with both hands
  • Explores objects by mouthing them
  • Follows a moving object smoothly across their full field of view

Language & Communication

  • Babbles with playful, sing-song sounds
  • Laughs out loud
  • Uses different pitches and tones to express different moods

Social & Emotional

  • Enjoys playing with people, may cry when play stops
  • Copies some facial expressions
  • Shows clear excitement when a familiar caregiver appears

Feeding & sleep at this age

Still milk-only for most babies at this stage. Readiness signs for solids — steady head control, sitting with support, visible interest in food — are worth noting now even though most guidance points to starting around 6 months. This is also a commonly reported age for a temporary sleep regression as sleep cycles mature and become more like an adult's, which can feel like a step backward even though it reflects real developmental progress.

When to check in with your pediatrician

Doesn't laugh or make squealing sounds, doesn't bring objects to their mouth, doesn't push down with their legs when feet touch a firm surface, has limbs that seem very stiff or very floppy, or doesn't seem to notice their own hands.

This is general information, not medical advice — if anything about your baby's development concerns you, talk to your pediatrician.

Questions about 4 months old

Can I start solid food at 4 months?

Some babies show readiness earlier, but most pediatric guidance recommends waiting until around 6 months unless a doctor advises otherwise for your specific baby. Signs of true readiness — not just watching food with interest, but sitting with support, strong head control, and losing the tongue-thrust reflex — matter more than the calendar date.

My baby rolled tummy-to-back but not back-to-tummy — normal?

Yes — the two directions often don't arrive together. Tummy-to-back tends to come first for a lot of babies, since it requires less coordinated effort, with the reverse roll following weeks or even a couple of months later.

Why does everyone talk about a '4-month sleep regression'?

Around this age, sleep cycles mature and start to resemble adult sleep architecture more closely, with more distinct light-sleep phases where babies partially wake between cycles. That can mean more night waking for a few weeks even though nothing is wrong — it's a sign of the sleep system developing, not regressing.

Is drooling a lot at 4 months a teething sign?

Increased drooling can start well before any visible tooth, sometimes months before, and isn't a reliable sign that a tooth is imminent on its own. It's common at this age regardless of teething timeline, partly because babies are also starting to explore objects — and their own hands — with their mouths more.
